1. Online application - this form is for students.
2. Parent/Guardian Consent - this form must be filled by a parent/guardian.
3. Payment - The BMC fee of $150 can be payed online to the MSRI.
4. Photo Release Form - The only form that can't be done online. It has to be printed and signed by a parent/guardian. Bring it to the next session of the math circle.

Although we suggest you to do the online application, you can apply by mail as well.
Download, print and fill these 3 forms (first one should be completed by a student, and the other two by parents):

1. Online application - to be filled by the student.
2. Parent/Guardian Consent - this form must be filled by a parent/guardian.
3. Payment - You can pay either online , or you can write a check. If you wish to pay with check, please address the check to "Mathematical Sciences Research Institute", and in the "Note" section at the bottom left of the check, clearly state the name of the student, the specific semester (Fall or Spring), and "...for the BMC".
4. Photo Release Form - This form has to be signed by a parent/guardian.
