Evaluation for the BMC 2016-2017

Instructions: This form should be filled by students, parents, teachers, and others involved with the BMC in 2016-2017 by May 1st, 2017 or earlier. Please note that students and parents must fill in the survey separately. The Evaluation From may be submitted by paper or by online.

Notes: We need your answers to the following questions in order to improve the circle, make it more visible, spread the math circle movement across the US, and apply for funds in order to keep BMC such a low-cost program to all students. Please answer these questions as best as you can. The survey is anonymous, and the results will be used only in the aggregate form.
